Biology and Advantage of Hyperthermia in Cancer Treatment

Abstract

|
Hyperthermia is one of the oldest treatment modality which was recorded on Egyptian papyrus, more than 5000 years ago. Hippocrates (470-377BC) described hyperthermia in his aphorism "Those who cannot be cured by medicine can be cured by surgery. Those who cannot be cured by surgery can be cured by fire(hyperthermia). Those cannot be cured by hyperthermia, they are indeed incurable." Development of radiation therapy and chemotherapy Pushed out hyperthermia from main cancer treatments, but overall 5 year survival is still remained on similar and unsatisfactory level since last 30 years, in spite of various strong modalities for cancer treatments. Now, there is no doubt that we need other modality for cancer treatment, in addition to major 3 treatments, such as surgery, radiation therapy, chemotherapy. Hyperthermia is one of very ideal cancer treatment modality which is more effective in cancer environments, rather than normal tissue, even though the effectiveness is very sensitive by operating techniques and conditions. It is the time we think again about hyperthermia as an essential treatment modality for cancer treatment. Hyperthermia has several biological effects on tumor cells as well as normal cells. 1)Hyperthermia kills living cells above 42t, by inactivating in a time- and temperature-dependent fashion. 2)Hyperthermia enhance the effect of radiation & chemotherapy, increasing in complete response rate by a factor of 1.5 to 2 as compared with radiation alone in local-regional recurrence of breast cancer, advanced neck node metastasis, and for cutaneous and peripheral lymphnode metastases from melanoma. 3) Even mild hyperthermia (38.5-41.5℃) may improves tumour oxygenation, this may be radiosensitizing effect in combined with radiation therapy, even though there is some debates. This is the era which we need 4th modality for cancer treatments. Hyperthermia is the ideal modality that gives more damages to cancer tissue than normal, which is unsolved problem in radiotherapy and chemotherapy. 1 hope we can destroy one barrier in cancer treatment by hyperthermia. This work is our duty, now.
